Member Health Services

Community Health Plan of Washington (CHPW) offers a variety of health services to address your patients’ whole health. We have free programs available to members with complex health conditions, and connect members with community resources to address everyday factors that affect their health.

Our Health Services team consists of both clinical and nonclinical staff. We will coordinate with you and your patients to create a good care plan.

Health Management

Health Management

Good health starts with a good plan of care. CHPW’s care coordination team includes care managers who will help members develop a health management plan with their specific health needs and goals in mind. These programs support the relationship between you and your patients and track how well the care plan is working by using clinical and evidence-based guidelines.Learn more about our health management approach

Case Management

Case Management

Case management is available to all CHPW members. It can be very helpful for members with complex or chronic medical conditions, often those who are receiving care for more than one medical condition. A case manager will be assigned to help the member maintain and improve their health. You can refer patients as a clinic or provider, or they may refer themselves.Learn more about case management services
